·        This form is for participants that have already signed a City release waiver for the Recreation Department. If a City release waiver has not already been signed, you must sign one at the Recreation Office before the participant can register. It is not necessary to use this form if you have registered with us since August 2005.

·        Please print and fill out all registration information completely. Questions, call 773-6458.

·        This form is to be used for programs offered by the City Recreation Department only.  Checks are made payable to the City of Satellite Beach unless otherwise noted.  Payments for some programs such as Soccer, Little League, Basketball, Football, etc. are made payable to that particular organization and requires a different registration process.  In most cases, the “Pay to” is noted with the program information.

·        Keep in mind that most programs fill up quickly!  If you use this form, call ahead to make sure there is space available.  A student is registered in the program when payment is received. How do I register?

q       Registration can be completed by making the class payment at the Satellite Beach Recreation Dept., 8:30am-6 pm M-F or by completing the above form and mailing it to our office.  To attend an activity in which there is no charge, call our office at 773-6458 to place the participant’s name on the program list.

q       When determining grade for your child, use the 2008-2009 school year.

How do I get a refund?

q       Call the Recreation Department, 773-6458 to make a refund request.  You must make your request directly to the Rec. Dept. staff (phone messages will not be accepted).

q       If you notify the Department about your withdrawal three business days prior to the start of the first class you will receive a full refund, unless otherwise noted.  If you notify the Dept. later than three days but before the 1st day of the class, you will receive a partial refund.  No refunds the day of or after the class has begun.

q       If a class/program is canceled, you will be notified of the change and a refund will be issued.  

q       Refund processing takes up to three weeks and you will receive a check in the mail.

NSF and Return Check Policy

q       The following fees will be assessed on any NSF or Returned Check.  Check writing privileges may also be suspended.

q       $25 fee- for checks up to $50; $30 fee – for checks from $50 - $300

q       $40 or 5% fee (whichever is greater) for checks greater than $300
